Payment Information

Patient Payment Responsibility
Most, if not all, insurance plans require a payment by the member in the form of a co-payment, coinsurance and/or deductible. The plan covers a specified amount or percentage provided that the member follows the guidelines or rules of the plan. It is the responsibility of the member to follow these guidelines. Any services – or portion of services – not covered by your plan are your financial responsibility. In the case of Workers’ Compensation, any services not covered by Workers' Compensation are your responsibility.

Coverage and Benefits Questions
The member should direct questions regarding coverage and benefits to the insurance company or agent. The insurance card is a source of valuable information, including telephone numbers to call for information and/or pre-authorization.

Filing a Claim
Our office will file a claim with your plan to assist you in obtaining coverage for the medical services you receive. As a patient, you will be required to bring your insurance card(s) to your appointments in order for our staff to file your claim.
