Being Prepared for Your Visit


 

What should you bring to your Orthopaedic Institute appointment?

  • Completed patient information form
  • Insurance card(s)
  • Identification
  • Workers compensation forms (if applicable)
  • Co-payment required by your insurance
  • Referral authorization (if required by insurance)
  • X-rays, MRI films or other diagnostic studies you have had for this problem, including written report of the results
  • A list of medications you are currently taking
  • Comfortable shoes and loose-fitting clothes
  • Wear shorts if you have a lower extremity or back problem
  • Wear a tank top or sleeveless top if you have an upper extremity problem
