We request that you complete these forms and return them to our office at your earliest opportunity. With a week or more until your appointment you may mail or fax (937-298-8260) the completed forms to our office. If your appointment is within a few days, please complete the forms and bring them with you on the day of your appointment. We ask that you arrive at least 15 minutes prior to your appointment time to register for your visit.
If you find it necessary to cancel an appointment, please do so as early as possible so that we are able to better accommodate you and other patients.
