Coronavirus Monitoring Questions
Have you tested positive for Covid 19?
- If the patient has tested positive and is now released from care, gain patient permission to obtain negative results from Provider prior to scheduling!
- If patient is Positive, they cannot be seen until they meet the CDC and State of Ohio guidelines.
Have you been around anyone who has tested positive?
Have you or anyone who lives with you traveled outside the State of Ohio?
Have you or a family member been in contact with someone who has traveled outside the State of Ohio?
If the answer is yes
The patient needs to be rescheduled for 30 days after exposure. They then need to be evaluated for upper respiratory symptoms before rescheduling.

- Monday: 7:30am — 5pm
- Tuesday: 7:30am — 5pm
- Wednesday: 7:30am — 5pm
- Thursday: 7:30am — 5pm
- Friday: 7:30am — 3:30pm
- Our Jamestown office is temporarily closed.
