Being Prepared for Your Visit
What should you bring to your Orthopaedic Institute appointment?
- Completed patient information form
- Insurance card(s)
- Identification
- Workers compensation forms (if applicable)
- Co-payment required by your insurance
- Referral authorization (if required by insurance)
- X-rays, MRI films or other diagnostic studies you have had for this problem, including written report of the results
- A list of medications you are currently taking
- Comfortable shoes and loose-fitting clothes
- Wear shorts if you have a lower extremity or back problem
- Wear a tank top or sleeveless top if you have an upper extremity problem
